The Czech Republic issues different types of visas to Pakistani visa applicants as well as visa applicants from Afghanistan because the embassy in Islamabad is also accredited to Afghanistan. What Documents are Required for Czech Republic Visa in Pakistan?

  17. Consent letter from the parent (if a minor is traveling with one parent)
  18. Business registration documents (for business people)
  19. Chamber of Commerce certificate (for business people)
  20. For civil servants NOC from the department
  21. Travel insurance for the entire stay (minimum coverage 30,000 Euro)

Note: Similar are the documents required for a short-term visa to Slovakia, because the Czech Republic also processes the Schengen visa for the Slovak Republic.

These are basic standard documents for a visit visa to the Czech Republic.
